Home Docs R Us cares for patients suffering from any illness or chronic condition in the safety of their home. We strive to ease those who have difficulty attending doctor office visits, as well as helping their families and caregivers. Our in-home services provide quality medical attention to each a patient individually.
Post Hospital home visits are used to prepare the patient and/or family for self-care and prevention of readmissions. This visit will also include a medication reconciliation that takes the hospital regimen into account. Our PCP or extender will ensure the patient understands the instructions on taking their medications, manage their condition and watch for any red flags that could indicate the condition worsening. Home Docs R Us will provide a contact number for caregivers, family members and patients to reach if there is a non-emergency after hours.
Annual Wellness visits are yearly appointments with your PCP to create and update a prevention plan. Home Docs R Us provides these visits in the comfort and privacy of your home. Our plan is to try and prevent any illness based on your current health and risk factors.

Care Coordination is a patient and family-centered, team-based activity designed to meet the needs of patients. Home Doc R Us evaluates each patient and provides what is necessary for the family and patient.
Apart from Care Coordination we provide Clinical Coordination services. Home Docs R Us works with different providers to ensure that each patient is managed and cared for. Home Doc R Us provides services that will meet the needs of each patient individually.
Activities of daily living (ADLs) are day to day activities someone can do without assistance. There are six basic ADLs we review: eating, bathing, getting dressed, toileting, transferring, and continence. Each of these will be reviewed by our physician or extender. This will help in determining what type of long term care a person will need.
